As in past years, the district school physician will need to review all student's medical eligibility forms. If your child is interested in playing an intramural sport at Pine Brook, there must be a medical eligibility form on file by the August due date from your child’s medical provider.  Please be aware that as per state guidelines, “the examination shall be conducted within 365 days prior to the first day of the athletic season and shall be conducted by a licensed physician, advanced practice nurse (APN), or physician assistant (PA).”  

Medical eligibility forms must be received by 8/16/24. Please keep a copy for your records. Incomplete forms will be returned. Make sure your physician signs, stamps, and completes all sections of the form. Forms must be mailed or dropped off in the drop box outside the school. Emailed submissions will not be accepted.

Health History Update Questionnaire - In order to participate in a school-sponsored intramural athletic sport, each student whose physical examination was completed more than 90 days prior to the first day of official practice shall provide an updated health history questionnaire. The questionnaire needs to be completed and signed by the student’s parent/guardian.
